PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.
I, the undersigned participant (or parent/guardian if participant is under 18), acknowledge and agree to the following terms in connection with my participation in the Sunnyvale International Church (SIC) Softball Program and the Bay Area Inter-Church Softball League (collectively, the "Activity"):
1. Assumption of Risk. I understand and acknowledge that participation in softball and related athletic activities involves inherent risks, including but not limited to: physical injury (including serious injury or death), property damage, collision with players or objects, exposure to weather conditions, and equipment failure. I voluntarily and knowingly assume all such risks.
2. Release and Waiver of Liability. In consideration of being permitted to participate in the Activity, I, on behalf of myself and my heirs, assigns, personal representatives, and next of kin, hereby fully release, waive, discharge, and covenant not to sue Sunnyvale International Church, DD Perez Ministries, their officers, directors, volunteers, employees, agents, representatives, sponsors, and any affiliated league or facility (collectively, "Released Parties") from any and all claims, demands, losses, liabilities, or damages arising out of or related to my participation, whether caused by the negligence of the Released Parties or otherwise.
3. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur as a result of my participation in the Activity.
4. Medical Authorization. In the event of an emergency in which I am incapacitated, I authorize the Released Parties to seek immediate medical attention on my behalf. I understand that I am solely responsible for all related medical costs.
5. Rules and Conduct. I agree to abide by all league rules, church conduct standards, and the directions of team leadership. I understand that conduct unbecoming of a Christian witness may result in removal from the team.
6. Photo/Media Release. I grant SIC and DD Perez Ministries permission to photograph or video record me during the Activity and use said media for church and ministry purposes, including but not limited to social media, websites, and promotional materials.
7. Acknowledgment. I certify that I am physically fit and have no medical conditions that would prevent my participation, except as disclosed in this form. I have read this release in its entirety and understand its terms.
This agreement shall be construed under the laws of the State of California.
